My colleagues Dov and David provided a great explanation to why images may be missing from a product feed in this thread:

To summarize their points, the images could be missing if you have changed their links on the backed, if they are out of stock, or if the product image has been updated after it was added to the feed.

I would recommend reading through that thread and investigated each of those possible causes/solutions as a first step.
