Hi,
I have recently joined klaviyo and published a contact form for the new subscribers with some questions,
I am wondering how I can send a campaign to the old contacts with a form to collect from them their information and interests and favourite brands.

The easiest way to gather more information about these pre-existing contacts through a campaign email would be to include a button or link that redirects users to a customized Manage Preference Page.
With this strategy in mind, you can send these pre-existing contacts a campaign email which has messaging in the body such as “Let us know what your interests and favorite brands are! Click the button below” with a button that has a link URL of {% manage_preferences_link %} to direct recipients to the manage preference page to select their interest and favorite brands.
