Can i use Sign up form as a survey form?

I want to create an exit intent form ( a survey form) for those that add a shopping cart but yet check out.

this is to collect both register or non register user feedback/survey why their abundance their cart. or a simple feedback.

Are we possible to do so ? or the form must have email address as requirement fields to be able for it to collect data?

can a list created without email address?

At this time you would either need an email address or phone number for the survey data to be added to. I will add a product feature request on your behalf to be able to get survey feedback anonymously
