Is your list set to double opt-in? This is the default when a list is created.

If so, a contact who submits your form will get counted as a form submission, but they won’t get added to your list until they confirm via clicking the link in the opt-in confirmation email.

Another consideration is that if a contact submits and confirms via double opt-in, they will be added to your list and the form submission count will increase by 1, However, if they submit that form again, they won’t be added to your list (already in it) and the form submission count will again increase by 1.

It’s worth setting ‘Don’t show to visitors in certain lists or segments’ and exclude your Welcome 15% list as they are already in that list and have the offer. That setting can be found in the form’s ‘Targeting and Behaviour’ settings.
