To create a segment, we are not able to filter between birthdate is set or not - any idea how to handle this?

Is the birthdate just day and month?
If so, for your campaign, you create a segment by including all months of the year (see below for Jan, Feb - just add all the other months as OR conditions). That will give you all profiles who have a populated birthdate property. Then use that segment as an exclusion to the campaign that asks contacts to add their birthdate.
For your list-triggered flow (new profiles), add the same set of conditions to a conditional split. Profiles who do NOT have their birthdate set will go down the NO branch. YES branch profiles exit the flow.
If you plan to have more than one flow email, add those conditions as a profile filter instead of multiple conditional splits - profile filters are evaluated before each email is sent.
