I have created a form in Kalviyo, called “Catalog Request” which has several fields to fill in.
Want to create a page in Shopify and call it “Catalog Request” and want to form to be in that page.
How do I exactly do this?
(from the articles I red, I paste the embeded code, starting with “<div” in the page html, but no form apprear!)

Make sure the form in klaviyo is live, because if it is not live in klaviyo you wont be able to see it on your website
