Is this a Klaviyo signup form you are trying to add these new fields to? If so, you should be able to do so by dragging in a Date block into your signup form. This would allow you to collect the date entered by your subscribers who fill out the form as a birthday profile property in order to trigger a birthday flow. If this was a third party signup form such as one provided by your Shopify theme or provided from a different app, I would suggest checking in on those sources regarding the ability to add more fields to them. If this were the case, keep in mind that in order for Klaviyo to recognize these fields as a date property, the date must be recorded in a specific format. Klaviyo is very specific on it’s data types and will only accept the following formats as a date data type which is further explained in the Acceptable Date and Timestamp Formats for Profile and Event Properties article.
