Email and Phone Number in one step

Hello!  I am having a very hard time finding a template where I can get a email and phone number in one step.  I saw a different forum that showed templates but I can’t find any of them.  I can only find forms that are name and email or name and phone number.  Can someone please help me find a template like this?  I tried to build one myself but I was not able to figure it out.  I attached an example photo.

To add some more strategy to @ArpitBanjara’s resolution above, the reason typically SMS consent is done separately (ie. in step 2, or with a dedicated targeted sign up form) is to make sure you capture consent for email first. 

Typically customers are more picky about what brands they allow them to send SMS messages. 

If you do go ahead with a combined popup, I would highly recommend running an A/B test against a form with 2 steps, or closely monitoring the form’s performance against the Klaviyo benchmarks.

Have you tried adding the “Phone number” block by dragging it into the form? below is how it should look. If you dont find the template, just drag and drop like I showed below
