I would recommend taking a look at the Signup Form Troubleshooting article to see if the suggestions within it can help locate a reason as to why your form isn’t displaying. 

The most common reason a form many not be appearing is typically due to your form’s behavioral settings, if the form is not set Live, if you have not installed either the Klaviyo onsite javascript or embed form code appropriately, or if there happens to be multiple Klaviyo onsite javascripts present on your site.
