Yes, it’s possible.

Create a Lego shape on a transparent background, trim to the transparent pixels and save as a png or gif. 

You will likely need to fill the inner part of the lego image with a colour, otherwise the screen content behind the pop-up will make your form fields difficult to see.

Import your Lego .png image as the form’s background image. You’ll end up with a form that has a Lego shape.

My poor example attempt below - needs tweaking - but you can see what it will look like.
