There are a couple of things that can be happening here, do you mind confirming some clues to get on the right track?
To be clear, is this a Klaviyo form that is misfiring or an external form? When you say the button doesn’t work, can you confirm that the button has proper coloring (not grayed out), and allows a user to submit their email? Additionally, you mention being able to see the ‘form submitted’ metric tracking, yet the users isn’t being added to the list? Can you confirm the $source for a profile who submits the form? It should be the form’s name if the form is working correctly.

You've probably already checked this but make sure the email address you're using is not signed up to the list already. You can only subscribe once, if you're already subscribed it won't trigger the welcome flow again. Might be a silly thing but seen this happening before.

If a profile has been deleted in klaviyo, can it then be added to a list again when it signs up using an embed form? I am struggling with a similar issue. I have a signup embed and email flow. I can’t test it since I’ve used my email address before. I deleted my profile and tried to test it. When I submit the form, the source property correctly assigns, but the email is not added to the list. It’s a new list so no one has ever been on this list.
I figured out the problem with the list. When I created a new list, it defaulted to double opt-in.
