When I visited your site on desktop, I was able to see the form (on the bottom-left of your homepage). Can you check again to see if it is populating for you? 

If not, this leads me to believe you have a browser, plug-in or firewall enabled preventing the form from populating. I would also try viewing the form from another browser and/or an incognito window to confirm.

Thank you @Dov, the last time I checked on my laptop, it completely disappeared but now it shows up again, that’s weird. Thanks for letting me know.
