For Klaviyo Embed form to show on your website first you need to publish the form in Klaviyo. From what i see you form is still in Draft mode and that is why it doesn’t show on the website. Turn the form live and give it few minutes and then refresh the page it should be there..

I have checked the source code where I added this “<div class="klaviyo-form-WcZsTR"></div>”. But In that page not showing any javascript code for Klaviyo...
Also I have checked the Klaviyo settings. but did not find out any javascript code.

I have added this code “<script async type="text/javascript" src="https://static.klaviyo.com/onsite/js/klaviyo.js?company_id=YOUR_ACTUAL_API_KEY"></script>” in footer.php. Now working.
