You’ll need to create a page, or use an existing page, to grab a url and for the QR to work in your use case.

A Klaviyo full page form can be set to trigger immediately and on a specific url . Example settings below

So you can take that approach and have the full page form open as soon as the user opens a browser from your QR code and hits that url.

If you want greater control over who sees that form, there are other targeting settings including triggering on a url parameter you add to your QR code url. For example if you add  ?utm_source=qr_code to your target url to generate the QR code, the full page form would only appear when the QR code is used ie with that source value included.
