I had followed the installation instruction and paste the relevant codes in my website.
When I load the page for the first time, the form will appear. However, when I refresh the page, the form will no longer load.
Anyone has any idea why?

Thanks for providing the URL for testing! I tested your form and it appeared immediately to me in Google Chrome. I was able to fill it out and refreshed it again and it loaded immediately. There are various reasons why an embed form won’t appear in your browser such as:
I would ensure you aren’t using either of these option to view your form as well as investigation these similar Community threads on a similar issue to gain more insight!
