How to create a customize a Klaviyo unsubscribe page like below?

Happy to help!
It would take some custom coding, but you can definitely create a page similar to your screenshot. While we can provide you with documentation on how to create a custom unsubscribe page, it will take custom coding that neither Support nor Community can help you set up. If you don’t feel comfortable custom coding on your own, you can reach out to a Klaviyo partner, which all of our Community Champions are, who would gladly assist in setting this up for you!
It should be noted that a feature like the one shown in the screenshot, which allows users the option to unsubscribe from any specific list, is only an option for paid Klaviyo users. Otherwise in free accounts, any unsubscribe, no matter if you create a custom page that gives a user options, will be recorded as a global unsubscribe in Klaviyo and they will no longer receive any marketing from you.
Additionally, I’d recommend checking out our Academy courses to help level up your Klaviyo knowledge!
