Add fields to Shopify Sign-up form

Hi!

I have Klaviyo connected with Shopify (with the App).

My sign-up form (in a pop-up) shows only “First Name”, “Last Name” and “Email” fields and populate a specific list in Klaviyo.

I want to add 2 fields : month and day for birthdays. I cannot find how to add these field (on Klaviyo side or on Shopify side) since I cannot edit this sign-up form.

Is this a Klaviyo signup form you are trying to add these new fields to? If so, you should be able to do so by dragging in a Date block into your signup form. This would allow you to collect the date entered by your subscribers who fill out the form as a birthday profile property in order to trigger a birthday flow. You can learn more about the Date block and how it functions in recording custom profile properties from the following Help Center articles:

If this was a third party signup form such as one provided by your Shopify theme or provided from a different app, I would suggest checking in on those sources regarding the ability to add more fields to them. If this were the case, keep in mind that in order for Klaviyo to recognize these fields as a date property, the date must be recorded in a specific format. Klaviyo is very specific on it’s data types and will only accept the following formats as a date data type which is further explained in the Acceptable Date and Timestamp Formats for Profile and Event Properties article.
