Signup form stuck — status "Live" but never renders on live site (no errors, all settings correct)

Hi all — hoping someone's run into this before.

I have a popup signup form that's set to Live, but it will not display on my actual storefront no matter what I try. Here's what I've already checked and ruled out:

  • Klaviyo script confirmed loading correctly on the site (correct company ID, verified in dev tools)
  • Zero JavaScript console errors
  • Form status double-checked as "Live" (caught it stuck in "Editing" once already and fixed that)
  • Triggers: exit intent, 20s delay, and "after 1 page seen" — set to OR, not requiring all
  • Targeting: set to show to everyone, no URL/location/UTM restrictions
  • Cookie consent accepted every time before testing
  • Tested in desktop Chrome incognito, republished and retested, and on a separate mobile device on cellular data (totally different network)

Still nothing — the form just never appears, on any device or network. No errors anywhere I can see, it just silently doesn't render.

Store is on Shopify, using the Klaviyo onsite embed via the app. Has anyone hit this before, or know of a known bug/edge case with forms getting "stuck" like this even after a clean republish? Would also take any tips on where else to look (Shopify-side conflicts, caching, anything) since I've run out of ideas on the Klaviyo side.
