Thank you for posting your question in the community. I am sorry to hear you are having issues with the Klaviyo form, I hope I can help.
A couple of questions:
What type of website are you trying to set up the form on? (Custom, Ecommerce, etc)
If ecommerce, what ecom platform are you using? (Shopify, Woocommerce, custom, etc.)
Depending on these answers, you may need to install the Klaviyo Script on the website in order to get the forms to display correctly. For example, if you are using Shopify and you’ve completed the integration with Shopify, the Klaviyo Script would already be installed, so troubleshooting would likely mean uninstalling the integration and trying again.
If you are using a custom website/non-ecommerce/etc. and you haven’t installed the Klaviyo Script, I would start there.
The Klaviyo Script looks like this:

The 6 highlighted ‘x’ would be replaced by your Klaviyo Public API Key which you would find in Klaviyo by navigating to your Settings > Account > API.
You’d then install this script in a container on your website so it is loaded on every page, like the header tag, for example.
The last step once you have the script on the website is to make sure that the form is Published in Klaviyo.
