Why is my Embed Form from Form Builders Missing Some Fields?

hi All

 

I created a form by Form Builder and embed it to a Shopify website via form embed code. 

Look the below image. the phone field some text block are missing.

 

I have try to look the Developer Tool of Chrome Browser. I got a message about bug of file https://static.klaviyo.com/onsite/js/vendors~View.6cd7a290b7db0f04ca18.js Error: [Parchment] Unable to create [object HTMLDivElement] blot

Just wanted to close the loop on this topic in case other members of the Community may be experiencing the same issue. 

It appears that this behavior of certain elements of the embedded form is not rendering is caused by a Javascript conflict that was present on the site.

To ensure that the Klaviyo Signup Form loads and behaves appropriately, you'll want to double check to make sure there aren’t any conflicting Javascripts present. 

I would also recommend reaching out to a developer if you're unfamiliar with that process or reaching out to members of our extensive network of agency partners.

Hi @dhbien.itprog, definitely an abnormal occurrence. I know that our engineers are taking a look into this and will be reaching out when they’ve been able to diagnose the issue.
