If I have an email flow and use the condition “has placed an order over all time”, does this also account for those customers who “has placed order since starting this flow” or do I need to add “AND” and include both profile triggers?

In the way you asked the question the answer is Yes. Placed order over all time contains people who placed order since starting this flow because placed order over all time is wider concept. Set up as mentioned it has no limitation so everyone that ever placed order will be counted in, while the other condition since starting this flow is only looking into people who placed an order from the moment of triggering the flow till the moment of the check, so it has time limitation.
