Opting Customers into a new list from an email

We are launching a new Loyalty program that we want to get existing customers to opt in to. Ideally I want to send an email out to all the current existing customers in our database informing them of the new program and include a link to a form. When completing this form the customer is automatically added to the Loyalty list in Klaviyo. 

I attempted to create a sign up form within Klaviyo but this was just a pop up on our website. Can klaviyo create an actual page with a form on it?
