Capturing extra information from customers with a survey style form in Klaviyo

We have a growing customer base but have limited knowledge on why they are rating us so highly. We would like to send an email with a simple 7 questions survey that we can then use to match with their customer records.

We use to use forms in Privy but not sure whether that can be done natively in Klaviyo or we need an integration. Any help would be appreciated.
