Metric Failing: Form completed by profile

Hi There :)

The Metric “Form completed by profile” stopped working a month ago on all our accounts.

We use the klaivyo form as a “succes page” for subscribers. For example when they click “No thanks to product reviews”. This metric this usually created is also used to trigger flows and segments.

Does anybody know why it could have stopped working (At the same time accross 11 accounts).
