You can use Klaviyo's review widgets to achieve this. Here's a general approach:

Navigate to the Reviews tab in Klaviyo. How to customize review widgets | Klaviyo Help Center
Customize your review widgets. Klaviyo offers several widgets, such as the Review Summary Widget and Product Reviews Widget, which can be customized to fit your needs.
Drag and drop the widget into your header or announcement bar in your Klaviyo template. Collecting reviews/NPS using a header link bar | Klaviyo Community
Customize the appearance of the widget to match your brand fonts and colors.
