You should be able to display/hide your forms based on the Source, Medium, Campaign, Term and ID UTM parameters. Are you not seeing those options when you click the UTM parameter dropdown?
Would you mind further clarifying the issue you’re seeing? Are utm_source, utm_term and utm_id not staying on the URL when clicking a link that leads to this page? Or is the form not showing up when expected at certain times? Once I have a bit more info, I’m happy to help out some more!
